Igarashi often gets fired because he doesn't seem to understand people's feelings. This brings him to work in a new hospital, Amakasu General Hospital. He meets Mr. Kikujima, a world renowned photographer, and tries to find the cause for his severe headaches when everyone else believes it's the rupture of an aneurysm. He also meets his childhood friend, Amakasu An who stood up for him when he was bullied during his childhood.

Fujimoto Naoki is found dead by his younger brother, Yuta, and the hospital wants to perform Ai (Autopsy Imaging) to determine the real cause of death. After performing Ai, Igarashi detects abnormalities in Naoki's liver and believes the cause of death to be an unusual one. The story takes an unexpected turn and it brings Naoki's stepfather, Mr. Fujimoto, to realise the truth that he never knew.
